POLICY: To offer services at a reduced price based on individual need and ability to pay.

PURPOSE: Front Range Clinic offers a sliding fee scale to ensure that all patients receive addiction care (medical and behavioral) regardless of their ability to pay. This policy reviews the income and family size to determine the appropriate payment options available. The Federal Poverty Guidelines are used to create and update the sliding fee scale and determine eligibility.

PROCEDURE: Patients seeking discounted services are required to complete the sliding fee program application and provide proof of income (last 3 months of paystubs), and proof of expenses. All approved applications must be updated every 6 months. No applications will be processed until all documentation is provided with a completed application. Front Range Clinic does not discriminate based upon the individual’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.

The discounts will be applied as follows:
