Porch Light Health (PLH) has been awarded $6,623,827 in first-year funding through Colorado’s Rural Health Transformation Program (RHTP), a federal initiative administered by the Centers for Medicare & Medicaid Services (CMS) to strengthen health care access in rural and frontier communities. The funding will expand coordinated addiction care, improve telehealth access, and support rural peer workforce development.

Statewide, Colorado distributed nearly $170 million to 91 applicants, supporting roughly 250 projects across 52 rural or frontier counties and three rural census tract counties. The award covers the first year of the five-year program.

Whole Person Health Project ($2.56 million): Serving Alamosa, Cañon City, Durango, Steamboat Springs and Sterling, this project adds patient navigation and connects addiction treatment, behavioral health, psychiatric care, infectious disease services and community resources. It also expands outpatient withdrawal management and on-site Hepatitis C confirmatory testing and treatment.

Telehealth Project ($3.79 million): Serving La Junta, Salida, Sterling, Walsenburg and Granby, this project links care-finding, scheduling, intake, treatment, medication access and engagement between visits, including direct access to MAT-on-Demand pathways. Staff remain available by phone for patients who prefer it or face technology barriers.

Peer Workforce Development ($281,382): Supports North Colorado Health Alliance’s expansion of its Peer Workforce Center of Excellence in Sterling and Logan County, helping rural peer professionals earn and keep their credentials.

“This is a historic moment for Porch Light Health and for the rural communities we serve.” — Doug Weiss, CEO, Porch Light Health
