Module Overview

Module 7 provides a comprehensive, evidence-based overview of Cannabis Use Disorder (CUD), addressing its evolving epidemiology, risk factors, and health impacts within the context of expanding legalization and increasing availability of high-potency THC products. Participants will deepen their understanding of the neurobiological mechanisms underlying cannabis addiction, tolerance, withdrawal, and craving, while strengthening their ability to apply validated screening and diagnostic tools in clinical practice. The module further equips clinicians to develop individualized, patient-centered treatment plans and coordinate longitudinal care, integrating behavioral interventions, harm reduction strategies, relapse prevention, and management of co-occurring psychiatric conditions across diverse care settings. 

Cannabis use patterns have evolved significantly in the context of expanding legalization, commercialization, and the availability of high-potency delta-9-tetrahydrocannabinol (THC) products. Although public perception increasingly frames cannabis as low risk or therapeutic, Cannabis Use Disorder (CUD) remains prevalent, particularly among adolescents and young adults. Early initiation, frequent use, and exposure to concentrated and vaporized formulations are associated with increased risk of dependence, cognitive impairment, psychiatric comorbidity, and functional decline. Despite these trends, many clinicians report limited formal training in identifying and managing CUD. 

Screening and diagnostic practices remain inconsistent across primary care, behavioral health, and addiction medicine settings. Validated tools—including the Cannabis Use Disorder Identification Test–Revised (CUDIT-R), Alcohol, Smoking and Substance Involvement Screening Test (ASSIST), and DSM-5 criteria—are available but underutilized. Knowledge gaps also persist regarding the neurobiology of cannabis addiction, including cannabinoid receptor type 1 (CB1) activation, dopamine modulation, tolerance development, and withdrawal physiology, particularly in the context of modern high-THC formulations and alternative routes of administration. 

Clinicians frequently report uncertainty in assessing withdrawal severity, evaluating relapse risk, and developing individualized, patient-centered treatment plans that integrate behavioral therapies, harm reduction principles, and management of co-occurring psychiatric conditions. Additionally, evolving legal and social contexts create complexity in counseling and longitudinal care coordination. 

This activity addresses identified gaps in knowledge, competence, and performance by providing evidence-based guidance on screening, diagnosis, neurobiological understanding, individualized treatment planning, relapse prevention, and coordinated longitudinal care for patients with Cannabis Use Disorder across diverse clinical settings. 

Global Learning Objectives

At the conclusion of this course, learners will be able to:

  1. Describe core principles of compassionate, patient-centered addiction care, including the practical application of harm reduction strategies in clinical settings.
  2. Explain key neurobiological mechanisms of substance use disorders and relate these mechanisms to patient behavior, relapse risk, and recovery trajectories.
  3. Select and use basic motivational interviewing techniques and validated screening tools to identify substance use disorders and inform initial clinical decision-making.
  4. Identify and initiate evidence-based pharmacologic and behavioral treatment options for substance use disorders, consistent with the learner’s professional scope of practice.
  5. Demonstrate an understanding of whole-person, team-based care by integrating healthcare, behavioral health, social services, and legal considerations into treatment planning.
  6. Apply practical, recovery-oriented strategies to respond to crises, relapse, and common real-world barriers encountered in early addiction treatment settings.

Module 7 Learning Objectives

At the conclusion of this module, learners will be able to:

  1. Describe the epidemiology, risk factors, and health impacts of Cannabis Use Disorder (CUD), including the effects of early initiation, high-potency products, and changing legal landscapes on prevalence and clinical risk.
  2. Explain the neurobiological mechanisms underlying cannabis addiction, tolerance, withdrawal, and craving, including CB1 receptor activation, dopamine dysregulation, and the impact of modern high-THC formulations and routes of administration.
  3. Apply validated screening and assessment tools (e.g., CUDIT-R, ASSIST, DSM-5 criteria) to identify CUD, determine severity, and evaluate functional impairment in clinical practice.
  4. Develop individualized, patient-centered treatment plans for CUD that integrate behavioral interventions, withdrawal management strategies, harm reduction principles, and treatment of co-occurring mental health conditions.
  5. Assess risk for cannabis withdrawal and relapse by evaluating use patterns, prior quit attempts, environmental triggers, and psychosocial supports, and implement evidence-informed strategies to mitigate these risks.
  6. Coordinate longitudinal care for patients with CUD by engaging family or support systems (with consent), leveraging community and peer-support resources, monitoring outcomes over time, and addressing legal, ethical, and social determinants of health.
